At Cove Recovery, Vivitrol is a tool — not a treatment on its own. We administer it as part of a structured, accountable program that addresses the full picture of opioid use disorder and alcohol use disorder: your substance use, your mental health, your recovery goals, and your life.

If you are receiving Vivitrol elsewhere with no therapy, no monitoring, and no real treatment support — we offer something different.

Who We Prescribe Vivitrol For

    Cove Recovery uses Vivitrol across several clinical pathways. Unlike some providers who administer a monthly injection with minimal support, we prescribe Vivitrol only within a fully integrated treatment program.

    We prescribe Vivitrol for:

    • Clients who have completed a medically supervised Suboxone taper and are transitioning to full abstinence from opioid medication
    • Clients who have already detoxed from opioids and are opioid-free, ready to begin long-term relapse protection
    • Clients who received Vivitrol while in an inpatient or residential program and are continuing care at the outpatient level
    • Clients with alcohol use disorder who are working toward abstinence from alcohol

    Why Cove Recovery is Different

    Many providers administer Vivitrol with minimal oversight — a monthly appointment, a shot, and little else. That is not treatment. At Cove Recovery, we will not simply give injections.

    Every client in our program must be actively engaged in treatment. That means:

    • Full SUD clinical assessment before medication begins — not after
    • Required participation in individual and group therapy
    • Co-occurring mental health screening and integrated treatment
    • Random urine drug screening — standard for every client, not optional
    • Regular prescriber oversight and medication management
    • Individualized treatment planning, updated as progress is made
    • Relapse prevention planning built into every treatment plan

    IMPORTANT
    Before starting Vivitrol, clients must be completely opioid-free for a minimum of 7 to 14 days. Beginning naltrexone while opioids are still present in the body can trigger sudden, severe withdrawal. Our medical team manages every transition carefully — no client is rushed.

    About Vivitrol (Naltrexone)

    Vivitrol is a monthly injectable form of naltrexone — an opioid receptor antagonist that blocks opioid and alcohol effects in the brain for a full 30 days. It is FDA-approved for both opioid use disorder and alcohol use disorder.

    Unlike Suboxone, Vivitrol contains no opioid component. It works by blocking opioid receptors entirely, which means opioids will have no effect while Vivitrol is active. This eliminates the reward of relapse and reduces cravings over time.

     

    Vivitrol helps:

    • Block the effects of opioids, eliminating the reinforcement of relapse
    • Reduce alcohol cravings and support abstinence from alcohol
    • Reduce opioid cravings over the course of treatment
    • Provide monthly accountability with no daily medication required
    • Remove the need for daily medication management — one injection per month
    • Support clients who have already achieved opioid-free status in maintaining it

    Vivitrol is most effective when combined with structured therapy and recovery support — which is exactly how we prescribe it.

    Vivitrol Pathways at Cove Recovery

    The choice of pathway is a clinical decision made with your treatment team — not a requirement before starting care. All pathways are fully supported within our program.

    Pathway 1
    Suboxone Taper to Vivitrol

    Bridge to Abstinence

    For clients who benefit from continued medication support as part of their long-term recovery. Suboxone is prescribed as a clinical tool fully integrated into active SUD and mental health treatment.

    Includes:

    • Medically supervised Suboxone taper at a pace determined by your clinical team
    • Careful monitoring throughout the taper and transition period
    • Individual and group therapy throughout
    • Monthly Vivitrol injection upon successful completion of the taper
    • Ongoing relapse protection — Vivitrol blocks opioid effects for 30 days

     

    Pathway 2
    Post-Detox Vivitrol

    Continuing Recovery After Detox

    For clients who have completed medical detox from opioids and are fully opioid-free, ready to begin structured outpatient treatment with Vivitrol as ongoing relapse protection.

    Includes:

    • Comprehensive SUD assessment at intake
    • Confirmation of opioid-free status before the first injection
    • Monthly Vivitrol injections administered by our medical team
    • Required individual and group therapy
    • Random urine drug screening

    Pathway 3
    Continuing Vivitrol After
    Inpatient or Residential Treatment

    For clients who were started on Vivitrol during an inpatient or residential stay and are transitioning to outpatient care. Cove Recovery ensures continuity — your injection schedule continues without a gap, and your treatment continues with real clinical support.

    Includes:

    • Seamless continuation of your Vivitrol injection schedule
    • Coordination with your previous inpatient or residential treatment team
    • Required individual and group therapy at the outpatient level
    • Individualized step-down treatment planning

     

    Pathway 4
    Vivitrol for
    Alcohol Use Disorder

    Vivitrol is FDA-approved for the treatment of alcohol use disorder and is prescribed at Cove Recovery for clients whose recovery goal is abstinence from alcohol — within the same structured, integrated clinical program.

    Includes:

    • Comprehensive SUD assessment addressing alcohol use history
    • Monthly Vivitrol injections to reduce cravings and support abstinence
    • Required individual and group therapy
    • Relapse prevention planning specific to alcohol use disorder
    • Random urine drug and alcohol screening

    NOTE
    Clients who are still actively drinking heavily may require medical evaluation for alcohol withdrawal before starting Vivitrol. Our team assesses this at intake and will coordinate referrals as clinically indicated.

    Integrated Mental Health Treatment

    Opioid use disorder rarely exists on its own. Depression, anxiety, trauma, and PTSD are common in people struggling with opioid addiction — and untreated mental health conditions are among the most significant drivers of relapse.

    At Cove Recovery, mental health treatment is not a referral or an afterthought. It is built into your treatment plan from day one.

    • Comprehensive mental health screening at intake
    • Individual therapy addressing trauma, anxiety, depression, and co-occurring conditions
    • Psychiatric services available as clinically indicated
    • Coordinated care between your prescriber, counselor, and therapist

    Treating substance use and mental health together leads to stronger, more sustainable outcomes. That is why we do both — for every client, on every pathway.

    Who We Serve

    We accept clients at multiple points in the recovery journey:

    • Individuals who have completed medical detox and are opioid-free, ready to begin Vivitrol
    • Clients transitioning from inpatient or residential treatment who were started on Vivitrol and need outpatient continuation
    • Clients currently on Suboxone who want a medically supervised taper with transition to Vivitrol
    • Clients with alcohol use disorder working toward abstinence
    • Clients with co-occurring mental health disorders requiring integrated treatment
    • Clients stepping down from a higher level of care into structured outpatient MAT

    Clients who are still actively using opioids will be comprehensively assessed and referred to medical detox or inpatient treatment before beginning Vivitrol. We coordinate that admission directly — we do not leave clients to navigate it alone.

    What to Expect in our Program


    Here is what the process looks like from the first contact through active treatment:

    2. Comprehensive SUD clinical assessment — we take the time to understand your full history, health, goals, and mental health needs.
    3. Level of care determination using ASAM criteria — outpatient MAT, or coordination of detox or inpatient treatment first if clinically indicated.
    4. Confirmation of opioid-free status before the first Vivitrol injection — your medical team will determine the right timing.
    5. Individualized treatment plan developed with your clinical team, including medication pathway selection.
    6. Active treatment begins — therapy, medication management, UDS monitoring, and mental health care, all coordinated within one program.
    7. Your plan evolves with you — treatment planning is ongoing and adapts as you make progress.

    Every client in our program participates in:

    • SUD group therapy — required for all clients
    • Individual counseling sessions
    • Mental health therapy and co-occurring disorder treatment
    • Random urine drug screening
    • Regular prescriber appointments and injection management
    • Relapse prevention planning


    In-Person and Telehealth
    — Hybrid Model

    Cove Recovery operates a required hybrid model. Initial assessments, prescriber appointments, and urine drug screenings require in-person attendance. Therapy sessions may be delivered in-person or via telehealth based on clinical need and client circumstances. Fully virtual medication management is not available.
